Job summary

Bgcarenav seeks a Hospice RN Case Manager to deliver comprehensive home-based care to patients in Harrison County, KY. You will assess needs, develop a holistic care plan, and coordinate services with families and the care team.

The role requires RN licensure in KY or a Compact State, CPR, and empathy for patients and families during end-of-life care. Candidates should be able to work autonomously within an interdisciplinary team.

Qualifications

• Graduate of an accredited school of registered nursing.
• Licensed Registered Nurse in KY or a Compact State.
• 1 year nursing experience preferred; new graduates considered.
• Strong clinical nursing skills; maintain professionalism in emotionally challenging situations.
• Compassionate, patient-centered care with respect and cultural sensitivity.
• Adheres to professional, ethical, and legal standards of nursing practice.
• Understanding of hospice philosophy and death and dying principles.
• Maintains CPR certification.
• Licensed driver with insured automobile meeting state minimum requirements.

Responsibilities

• Provide hospice nursing care in home or patient home settings.
• Create and implement holistic plan of care with patient goals and preferences.
• Advocate for patients and coordinate care for patients, families, and caregivers.
• Manage symptoms through pharmacologic and non-pharmacologic interventions for comfort, function and safety.
• Conduct frequent monitoring and follow-up to evaluate treatment efficacy and offer emotional support.

Position Summary:

The Hospice RN Case Managerdeliverscompassionate and comprehensive health care services to Hospice patients in a home care setting,orwhereverthe patient calls home.Home care settingsmayconsist ofa private residence, skilled nursing facility, personal care home, and/or other interim livingarrangements.The Hospice RN Case Manager’sgoal for excellentpatient care standardsbegins with active listening to create and implement a holistic plan of care based on the patient’s goals andpreferences.The Hospice RN Case Manager serves as the patient advocate to support, guide, and coordinate care for patients, families, and caregivers as they navigate Hospice Care.The plan of care will include managing symptoms throughpharmacologic and non-pharmacologic interventions foroptimalcomfort, functionality, and safety. Frequent monitoring and follow-up arerequiredto evaluate the efficacy of treatments, plans of care, and to offer emotional support to both the patient and the caregiver. A Registered Nurse in this position will havestrong communicationskills, the competence to work autonomously, professionalism to work within an interdisciplinary team, and the ability to provide care with empathy. Position Qualifications:
