Job summary

Chapters Health System is seeking an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the IDT, and provide clinical and palliative care to keep patients in their homes when possible.

Responsibilities include home visits, education on disease and safety, and collaboration with physicians and staff. A valid RN license and English communication are required, with potential emergency reporting as needed.

Qualifications

• Current RN license in the state where employed
• Minimum 1 year nursing experience; hospice or hospital preferred
• PACE locations may require Alzheimer's & Related Dementias Training
• Experience with EMR/EHR systems
• Mobile Driver—valid license and automobile insurance per policy
• Reliable transportation to meet visit schedule
• Ability to communicate effectively in English (verbal and written)
• Ability to visit participants in their homes
• Physical requirements including lifting and other tasks with or without accommodation
• Active BLS for healthcare professionals (AHA or Red Cross)

Responsibilities

• Provides and manages direct care to patients and families as part of the Interdisciplinary Team (IDT)
• Coordinates the Plan of Care reflecting evolving patient needs
• Educates patients, families and caregivers about disease, palliative care and safety
• Performs home visits to assess safety and medication compliance
• Communicates changes in patient condition to IDT; SBAR documentation
• Attends daily IDT collaboration meetings and provides reports
• Supervises Home Health Aides as requested
• May be required to report to work during emergencies to ensure continuity of services
