Job summary

Serenity Hospice Care Pennsylvania is seeking an experienced Registered Nurse (RN) Case Manager to provide compassionate hospice care. This role focuses on patient-centered plans, coordination with physicians and social workers, and ongoing education for patients and families.

The RN will ensure timely documentation, address barriers to care, and contribute to seamless care transitions within a supportive team environment.

Qualifications

• Current licensure as a Registered Nurse in the state of Pennsylvania.
• Strong clinical knowledge and skills, with a focus on hospice and palliative care.
• Excellent communication and interpersonal skills with patients, families, and professionals.
• Strong organizational and prioritization skills to manage multiple tasks.
• Ability to work in a fast-paced environment and adapt to changing priorities.

Responsibilities

• Perform initial and ongoing assessments of patients and their families to develop individualized plans of care
• Coordinate care with the interdisciplinary team, including physicians, social workers, and other healthcare professionals
• Provide ongoing patient and family education and support to ensure optimal care and quality of life
• Identify and address barriers to care, including psychosocial and spiritual concerns, and develop strategies to overcome them
• Collaborate with the hospice team to ensure seamless care transitions and effective communication
• Maintain accurate and timely documentation of patient care and communication with the healthcare team
