Serenity Hospice in Chester County, PA is seeking an experienced Registered Nurse Case Manager to join our team, overseeing home-based hospice care and coordinating the interdisciplinary team for patients and families in our community.

The RN will provide expert clinical care, symptom management, education, and emotional support while coordinating with physicians, social workers, and chaplains to ensure high-quality end-of-life care in homes and facilities.

Qualifications

• Current Pennsylvania RN license in good standing.
• Minimum of 1–2 years of nursing experience; hospice preferred but not required.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Compassionate, patient-focused approach with a commitment to dignity and comfort.
• Reliable transportation, valid driver’s license, and auto insurance.
• Ability to work independently while collaborating with a supportive team.

Responsibilities

• Perform initial and ongoing assessments of patients and families to determine hospice needs
• Develop, implement, and update individualized plans of care
• Provide skilled nursing care, pain and symptom management, and medication administration

Schedule: Full-Time – Monday through Friday – 8:30am-4:30pm

No Nights No Weekends No On-Call Required

About the Role:

Serenity Hospice is growing and seeking an experienced and compassionate Registered Nurse Case Manager to join our dedicated team serving patients and families in Chester County, PA.
