Job summary

Assisted Home Health & Hospice is seeking a Hospice RN Case Manager for a full-time role in Phoenix/West Valley. You will evaluate patient eligibility for services, develop comprehensive care plans, and coordinate with multidisciplinary teams to ensure high-quality hospice care.

The ideal candidate is a registered nurse licensed in Arizona, with CPR and clearances, ready to support patients and families through serious illness with compassion and expertise.

Qualifications

• Graduate of an approved school of professional nursing.
• Current, active license in the state of Arizona Board of Nursing.
• CPR certification and health/TB clearances.

Responsibilities

• Apply admission and re-certification criteria to establish patient eligibility and levels of care.
• Assess the impact of illness on physical, functional, safety and psycho-social needs.
• Develop individualized pain management plans for patients.
• Evaluate caregiver ability to meet patient needs at start and during care.
• Create palliative care plans based on identified needs and relief measures.
• Refer to additional duties as outlined in the Job Description.

What you will do:

• Apply specific criteria for admission and re-certification to establish patient’s eligibility and appropriate levels of care.


• Assess the impact of the serious illness or terminal diagnosis on patient’s physical, functional, environmental, safety, ADL and psycho-social needs, including risk for pathological grief.


• Assess all aspects of the patient’s pain and develop an individualized pain management plan.


• Identify patient verbal and non-verbal communications patterns.


• Assess the ability of the caregiver to meet the patient’s immediate needs at start of care and throughout care.


• Develop an individualized palliative plan of care based on the needs identified which demonstrates management of discomfort and provision of symptom relief.

Who you are:

• Graduate of an approved school of professional nursing.
