Accord Hospice of the White Mountains is seeking a compassionate Hospice RN Case Manager to join our growing team in Arizona. You will work under the direction of medical leadership while building meaningful relationships with patients and families at end-of-life.

The role emphasizes patient-centered care, education, and coordination with an interdisciplinary team. The position offers a Monday–Friday schedule (typically 8:00 a.m.

Qualifications

• Active RN license in Arizona (Required).

Responsibilities

• Assess patients' physical, emotional, and psychosocial needs.
• Educate patients, families, and caregivers on disease progression and care plans.
• Manage pain and symptoms to maximize comfort and quality of life.
• Coordinate care and serve as primary case manager for assigned patients.
• Collaborate with an interdisciplinary team including physicians, social workers, chaplains, CNAs, therapists, and volunteers.
