Job summary

Assisted Home Health & Hospice is seeking a Registered Nurse to cover on-call nights and weekends for our Phoenix-based Hospice Team. The role is an on-call position for Fridays through Sundays with on-call rates and per-visit pay outlined in the listing.

The RN will assess patient eligibility, manage pain, develop care plans, and coordinate with caregivers to ensure quality end-of-life care. Current AZ RN license and CPR are required.

Qualifications

• Graduate of an approved school of professional nursing.
• Current, active license in the state of Arizona Board of Nursing.
• Current CPR, health clearance, TB clearance.

Responsibilities

• Apply admission criteria and re-certification to determine eligibility and levels of care.
• Assess impact of illness on patient’s physical, functional, safety and psychosocial needs.
• Assess pain and develop an individualized pain management plan.
• Identify patient verbal and non-verbal communication patterns.
• Assess caregiver ability to meet patient needs at start of care and during care.
• Develop an individualized palliative care plan based on identified needs.

Hospice RN [On-Call Nights & Weekends ]- West Valley

• Phoenix, AZ

We are hiring for an RN to do on call Friday - Saturday – Sunday for our Hospice Team in Phoenix! This is an On-Call Position for Weekends. On-Call Rates: $50 On-Call Rate for P.M.s weekday and $100 for Sat/Sun's.

• RN On Call Eval - $200.00 per visit plus $10.00 car visit for mileage/phone allowance
• RN On Call Routine - $105.00 per visit plus $10.00 car visit for mileage/phone allowance

What you will do:

• Apply specific criteria for admission and re-certification to establish patient’s eligibility and appropriate levels of care.
• Assess the impact of the serious illness or terminal diagnosis on patient’s physical, functional, environmental, safety, ADL and psycho-social needs, including risk for pathological grief.
• Assess all aspects of the patient’s pain and develop an individualized pain management plan.
• Identify patient verbal and non-verbal communications patterns.
• Assess the ability of the caregiver to meet the patient’s immediate needs at start of care and throughout care.
• Develop an individualized palliative plan of care based on the needs identified which demonstrates management of discomfort and provision of symptom relief.
