Job summary

ExcelaCare Hospice in Big Spring, TX seeks an RN Case Manager to deliver hospice care in patients’ homes or facilities. You will coordinate with a multi-disciplinary team, schedule and perform nursing visits, and support patient and family through end‑of‑life care.

The role requires a valid RN license and CPR certification, with hospice experience preferred. A $2,500 sign-on bonus is offered and on-call rotation is part of the schedule.

Qualifications

• Graduate of an accredited school of nursing.
• Two years RN experience; hospice preferred.
• Current valid RN license; CPR certification.
• Driver’s license and auto liability insurance; not listed on OIG Excluded Providers.

Responsibilities

• Administers hospice care per physician orders and Plan of Care.
• Coordinate care with interdisciplinary team and family.
• Perform nursing assessments and document status and interventions.
• Educate patients and families on care plans and end-of-life options.
• Participate in on-call rotation and scheduled visits.
• Maintain confidentiality and comply with agency policies.
