Job summary

Phoenix Home Care and Hospice in Joplin, MO seeks an RN Case Manager to provide compassionate nursing care to clients with terminal diagnoses. You will work with the interdisciplinary team to develop and implement a patient/family centered plan of care, with goals of care discussions and guidance on healthcare decisions to improve quality of life.

Qualifications

• Missouri or Compact RN license is required.
• Valid driver's license and auto liability insurance.
• Experience in hospice or patient-family education is preferred.

Responsibilities

• Provide education to patients/family regarding the plan of care and healthcare decisions.
• Consult with the attending physician and Hospice Medical Director about changes in condition and plan of care.
• Coordinate hospice care with the team and families to ensure needs are met.
• Conduct initial and ongoing comprehensive assessments.
• Perform hospice care evaluations and informational visits.
