Job summary

Preferred Hospice of Missouri is seeking a Full Time RN Case Manager to serve patients in the Joplin, MO area. The role involves traveling to patients' homes, facilities, and other residences within a defined geographic area, with autonomy and teamwork as core aspects of daily work.

The RN will perform assessments, manage symptoms for terminal illnesses, supervise LPNs/CNAs, and coordinate care. Missouri RN licensure, reliable transportation, and EMR proficiency are required.

Qualifications

• Must be currently licensed as an RN in Missouri.
• Experience with EMR charting and telephony/ smartphone apps is a plus.
• Excellent communication and teamwork are essential.

Responsibilities

• See patients in private homes, assisted living, SNFs, or other residences.
• Perform head-to-toe assessments and provide symptom management for terminal illness.
• Supervise LPNs and CNAs; monitor medications and DME needs; assist with wound care.
