Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 2+ years of full-time experience working as an RN, or equivalent clinical experience in hospice care.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Make initial nursing evaluation to determine eligibility for hospice services.
• Identify and reassess patient's/family's needs every 15 days.
• Initiate and coordinate the plan of care.
• Document goals and patient responses to hospice care.
• Collaborate with the care team for daily patient care.
• Instruct patient/family in self-care techniques.
• Maintain accurate clinical notes on patient condition.
