Job summary

Qualifications

• Licensed RN in Louisiana with current license.
• Graduate of an accredited nursing program.
• Minimum two years of RN experience; hospice experience preferred.

Responsibilities

• Perform initial, comprehensive and ongoing patient/family assessments.
• Develop and implement care plans with goals based on nursing diagnoses.
• Administer medications and treatments per physician orders.
• Coordinate patient care as Case Manager when assigned.
• Provide education and emotional support to patients and families.
• Participate in on-call and holiday coverage as required.
• Document clinical notes and communicate with physicians and care team.
