Redcrest Hospice LLC in Bloomington, IN seeks a Registered Nurse Case Manager (Hospice) to provide and plan professional nursing care under physician direction, delivering direct patient care and documenting patient conditions. The RN I administers treatments and medications and coordinates multidisciplinary care to support comfort and symptom control.

Qualifications

• Valid RN license in the state(s) where service is provided.
• Bachelor's degree in nursing or healthcare-related field preferred.
• Experience in hospice or home health nursing is a plus.

• Provides skilled nursing care in a patient's home as ordered by the attending physician.
• Observes and monitors patient conditions and documents care.
• Develops an individualized plan of care and coordinates with multidisciplinary team.
