Job summary

Redcrest Hospice LLC is seeking an experienced RN Case Manager for our Muncie, IN service area. The role leads patient assessments and plans of care, coordinates multidisciplinary teams, and ensures quality, cost-effective end-of-life care in patients’ homes.

The RN will conduct initial and ongoing visits, update physician orders, and communicate changes to families. Indiana licensure and hospice experience are preferred; a valid driver’s license is required.

Qualifications

• 1 year of RN experience in acute care or home care settings.
• Hospice experience preferred.
• Maintains active RN license in state of employment.

Responsibilities

• Oversee overall patient care from start to end of life.
• Perform initial, comprehensive, and ongoing skilled nurse visits.
• Update doctor orders in patient chart.
• Notify doctor of significant changes in patient condition.
• Coordinate interdisciplinary care and supervise care providers.
• Provide end of life care in patient’s home as ordered.
• Maintain plan of care and communicate with families.
• Document all treatments and education per guidelines.
• Participate in on-call coverage and case conferences.
