Job summary

Amedisys seeks an experienced RN Case Manager to deliver home health and hospice services across the assigned area. You will assess patients, develop care plans with the care team, and coordinate referrals to ensure timely, high-quality care at home.

Qualifications

• One year of nursing experience.
• Current state RN license.
• CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and develops plan of care with the care team.
• Makes referrals to hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
