Job summary

Phoenix Home Care & Hospice is seeking an RN Case Manager in St. Louis, MO, full-time, Monday–Friday, days. You will work with an interdisciplinary team to develop and implement patient/family centered care plans, support end-of-life decisions, and enhance quality of life in home hospice settings.

The role requires Missouri- or compact-licensed nurses, a valid driver’s license, and strong leadership, organizational, and communication skills. A sign-on bonus and comprehensive benefits are offered.

Qualifications

• RN license required (Missouri or compact).
• Valid driver’s license.
• Strong interpersonal and organizational abilities.

Responsibilities

• Educate patients and families about the plan of care and healthcare decisions.
• Consult with the attending physician and Hospice Medical Director about changes to the plan of care.
• Coordinate hospice care with the team to ensure patient needs are met.
• Perform initial and ongoing comprehensive assessments.
• Conduct evaluations for hospice care and provide informational visits.
