Job summary

Phoenix Home Care & Hospice in Missouri is seeking an RN Case Manager to provide compassionate hospice nursing care to patients and families in the Springfield area. You will educate patients and caregivers on the hospice process, plan of care, and guide families through goals-of-care discussions with empathy and clarity.

You’ll collaborate with an interdisciplinary team, conduct comprehensive assessments, coordinate care, and communicate updates to ensure patient and family needs are fully met.

Qualifications

• Active Missouri RN license or Compact RN required.
• Hospice or Long-Term Care nursing experience preferred.
• Strong assessment, communication, and care coordination skills.
• Valid driver's license and auto liability insurance.

Responsibilities

• Provide compassionate hospice nursing care to patients and families.
• Educate patients and caregivers on the hospice process and plan of care.
• Guide families through goals-of-care discussions and healthcare decisions.
• Conduct initial and ongoing comprehensive assessments.
• Perform hospice evaluations and informational visits.
• Collaborate with physicians, Hospice Medical Directors, and interdisciplinary team members.
• Coordinate care to ensure patient and family needs are fully met.
• Communicate changes in patient condition and update plans of care as needed.
