Concho Hearts Hospice is currently seeking a dedicated RN Case Manager to join our Tomball, Texas team. This full-time role focuses on coordinating care for hospice patients, conducting comprehensive nursing assessments, and creating individualized plans that emphasize comfort and family education.

The RN will collaborate with the interdisciplinary team and supervise LVNs as appropriate, traveling throughout Tomball and surrounding communities to provide compassionate, high-quality care.

Formación

• Active Texas RN license or NLC license.
• Hospice, home health, SNF, or acute care experience preferred.
• CPR certification.
• Ability to travel throughout Tomball and surrounding communities.

We're looking for a dedicated and compassionate Full-Time Hospice RN Case Manager to join our growing team serving Tomball, Texas, and surrounding communities.

What You'll Do:

• Manage and coordinate care for an assigned caseload of hospice patients.
• Complete comprehensive nursing assessments and routine patient visits.
• Develop, implement, and update individualized plans of care.
• Provide skilled nursing care, pain management, and symptom control.
• Monitor changes in patient condition and communicate with physicians and the interdisciplinary team.
• Educate patients and families about medications, disease progression, comfort measures, and end-of-life care.
• Coordinate medications, medical equipment, supplies, and other patient needs.
• Supervise and collaborate with LVNs and Hospice Aides as appropriate
• Participate in interdisciplinary team (IDT) meetings.
• Complete, accurate, and timely documentation in the EMR.
• Provide compassionate guidance and emotional support to patients and families.

What We're Looking For:

• Active Texas Registered Nurse (RN) license or qualifying Nurse Licensure Compact (NLC) license.
• Previous nursing experience in hospice, home health, skilled nursing, or acute care preferred.
• Hospice RN Case Management experience is a plus!
• Strong clinical assessment and critical-thinking skills.
• Excellent communication, organization, and time-management skills.
• Ability to independently manage a patient caseload while collaborating with the clinical team.
• Compassionate, dependable, and committed to exceptional patient care.
• Current CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to travel throughout Tomball and the surrounding communities.
