Job summary

Concho Hearts Hospice is seeking a Hospice RN Case Manager to oversee a Tyler-area caseload, delivering skilled nursing care in homes and facilities. You will perform routine visits, admissions, and recertifications, while developing individualized care plans and collaborating with the interdisciplinary team to support patients and families.

This role emphasizes compassionate, patient-centered care with travel to local patients, documentation on a company tablet, and a supportive leadership

Qualifications

• Active Texas RN license required.
• Strong assessment and critical thinking skills essential.
• Excellent communication and teamwork abilities.
• Ability to work independently in the field and manage a caseload.

Responsibilities

• Manage a caseload of hospice patients in the Tyler area.
• Provide skilled nursing care in patients’ homes and facilities.
• Complete routine visits, admissions, and recertifications.
• Develop and update individualized care plans.
• Collaborate with physicians and the interdisciplinary hospice team.
• Provide education and emotional support to patients and families.
• Maintain accurate documentation using a company-provided tablet.
