Job summary

Suncoast Hospice of Hillsborough, a member of Empath Health, is seeking an RN Case Manager in Tampa, FL. The role supports a concentrated service area (Odessa, Westchase, Lutz, Citrus Park) with a focus on compassionate end-of-life care.

You will lead care coordination, develop Plans of Care with the IDT, and guide patients and families through the hospice journey. Qualified candidates hold a Florida RN license and an AA degree or equivalent; one year of related experience is preferred, and

Qualifications

• Experience in coordinating care with an interdisciplinary team (IDT).
• Ability to obtain patient history and perform age-specific assessments.
• Spanish/English bilingual preferred.

Responsibilities

• Obtain patient history and conduct assessments as per policy.
• Develop and update Plan of Care with patient, family and IDT.
• Coordinate care and supervise related staff and home health aides as needed.
• Provide patient/family teaching and document medications as prescribed.
