Job summary

Millenia Hospice is seeking an experienced RN PRN Case Manager to deliver compassionate end-of-life care across the Dallas–Fort Worth area. You will assess patients, plan individualized care, and coordinate with an interdisciplinary team to ensure comfort and dignity for patients and families.

Ideal candidates hold an active Texas RN license, have hospice or home health experience, and possess strong communication and time-management skills.

Qualifications

• Current and unrestricted RN license in Texas.
• 1–2 years hospice, home health, or related experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing care and symptom management.
• Coordinate care with physicians, social workers, chaplains, and other team members.
• Educate patients and families on disease progression, medications, and care techniques.
• Monitor patient condition and adjust care plans as needed.
• Maintain accurate and timely clinical documentation.
• Participate in on-call rotation as needed (PRN).
• Ensure compliance with hospice regulations and standards.

Key Responsibilities:

• Excellent communication and interpersonal abilities
• Ability to work independently and manage time effectively
• Reliable transportation and valid driver’s license
• Current CPR certification (required)

The salary is Negotiable Compensation: $100.00 per visit
