Job summary

Adoration Health is seeking an RN Case Manager for Hospice in Parkersburg, WV. You will deliver patient-centered care at home or in facilities, coordinating services and supporting families on the end-of-life journey.

Responsibilities include assessing needs, creating care plans, administering treatments, managing pain, educating families, and maintaining documentation. Requirements: WV RN licensure, 1 year nursing experience (3+ preferred), hospice knowledge.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patients wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient's family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
