Job summary

Adoration Health is seeking a Nurse Practitioner for Adoration Hospice in Parkersburg, WV. You will provide compassionate hospice care, conduct assessments, develop care plans, and coordinate with an interdisciplinary team to meet patient goals.

The role requires NP certification, a Master’s degree, and current state license. On-site responsibilities include documentation, education, and ongoing quality improvement within a supportive team.

Qualifications

• Graduate of an accredited School of Nursing and Nurse Practitioner program.
• Master’s degree with a minimum of one year NP experience.
• Current nursing license in the state of practice and NP certification.

Responsibilities

• Provide compassionate NP care and symptom management.
• Perform assessments, order/interpret diagnostic tests, and develop care plans.
• Prescribe medications, manage treatments, and coordinate hospice services.
• Conduct and document face-to-face visits for eligibility and recertification.
• Collaborate with physicians and IDT team to align care with patient goals.
• Educate patients & families on medical, emotional, and spiritual needs.
• Maintain accurate clinical documentation and ensure regulatory compliance.
• Contribute to quality improvement and professional development.
