Job summary

Adoration Health is seeking a passionate Hospice RN Case Manager to join our Parkersburg, WV team. You will provide 1:1 patient care, manage pain and symptoms, and develop care plans aligned with patient goals.

The role requires graduation from an accredited nursing program with a current RN license and CPR certification. Experience in hospice or similar settings is preferred, with opportunities for growth and stable employment in a supportive environment.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Possess and maintain CPR certification as required.

Responsibilities

• Assess/ monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient’s wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
