Adoration Health in Parkersburg, WV is seeking a Registered Nurse to join our hospice team as a Case Manager. You will deliver patient-centered care, coordinate plans, and work within an interdisciplinary team to support patients and families.

The role requires RN licensure, 1 year of nursing experience (3+ preferred), knowledge of hospice philosophy, and driving access with CPR in states where required. Schedule is Monday–Friday, 8am–5pm.

Formación

• Graduate of an accredited nursing program with current licensure to practice in state of operation.
• 1 year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured vehicle; CPR certification in states where required.

Responsabilidades

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient families and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate documentation.
• Participate in on-call rotation as required by local branch.
