Qualifications

• Graduate of an accredited school of nursing.
• Current and valid RN license in the state of New Mexico.
• At least 1 year of nursing experience; hospice or end-of-life care preferred.

Responsibilities

• Conduct thorough assessments of patients' needs.
• Develop individualized care plans with the team.
• Provide skilled nursing care including symptom management.
• Coordinate resources and referrals for patients.
• Offer education and emotional support regarding end-of-life care.
• Maintain accurate clinical documentation.
• Ensure compliance with healthcare laws and organizational procedures.
• Supervise direction to LVNs and HCAs.
• Participate in case conferences and quality assurance activities.
• Make scheduled home visits and be on-call.
