Ennoble Care is hiring a Full-time Hospice Registered Nurse Case Manager to serve the Arlington County region in Virginia. The RN Case Manager coordinates care under the Director of Nursing and a multidisciplinary team to maximize comfort for patients and support for families.

The role requires an active VA RN license, at least two years of nursing experience (hospice preferred), strong assessment skills, and reliable transportation.

Qualifications

• Must hold Virginia RN license.
• At least two years of professional nursing experience, hospice experience preferred.
• Ability to assess patient and caregiver needs in diverse settings.
• Strong teamwork within an interdisciplinary care team.

Responsibilities

• Manage a patient caseload across nursing process stages (assess, plan, implement, evaluate).
• Coordinate care with attending physicians, Hospice staff, and other agencies.
• Document accurately in electronic medical records and participate in Interdisciplinary Group meetings.
• Provide caregiver education and bereavement support as needed.
• Ensure continuity of care and effective resource use for patients and families.
