Job summary

Ennoble Care is hiring a Full-Time Admissions Nurse to access patients in the Arlington County, VA region and surrounding areas. An interdisciplinary team approach guides skilled nursing assessments, planning, and care to maximize comfort and health for patients and families.

The role reports to the Director of Nursing and collaborates with the attending physician and IDG to coordinate care, document plans, and ensure timely interventions in a home-based setting.

Qualifications

• Associate's Degree required.
• Licensed as a Registered Nurse (RN) in Virginia.
• Experience in hospice, home health, or geriatric nursing preferred.
• Must have reliable automobile and valid driver's license with minimum auto insurance.

Responsibilities

• Assess patients for hospice admission criteria.
• Assess physical, psychological, social and spiritual needs with IDG.
• Coordinate with physician, IDG and other agencies to plan and implement care.
• Document plan of care and update records as patient status changes.
