Job summary

Jobot is seeking an RN Case Manager in Muncie, IN for a Monday–Friday day shift with a company car and generous benefits. You will provide nursing care in patients' homes and facilities, and coordinate with the interdisciplinary team.

The role emphasizes compassionate hospice care, developing care plans, administering treatments, documenting in EMR, and supporting patients' families. Active RN license required; benefits include mileage support, full medical/dental/vision, and tuition

Qualifications

Active RN license in the state of practice.
Experience in hospice or home health preferred.
Ability to coordinate with interdisciplinary team.

Responsibilities

Provide nursing care to patients in hospice care in patient homes and facilities.
Develop and implement individualized care plans for patients and their families.
Coordinate with interdisciplinary team members to provide comprehensive care to patients.
Administer medications and treatments as prescribed.
Monitor and assess patient symptoms and provide comfort measures as needed.
Educate patients and their families on end-of-life care and support.
Document patient care and progress in electronic medical records.
Provide emotional and spiritual support to patients and their families.
Advocate for patients and their families to ensure that their needs are met.
Participate in on-call rotation as needed.
