Castle Senior Living, Inc. is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our growing hospice team in New Berlin, Wisconsin. You will assess patients and families, develop individualized care plans, and coordinate nursing care with the Hospice Director of Nursing.

Qualifications

• Current certification as a Registered Nurse (RN) licensed in Wisconsin.
• Excellent communication and problem-solving skills with compassionate bedside manner.
• Valid driver's license and auto insurance.

Responsibilities

• Provide skilled nursing care and support to patients and families where they call home.
• Perform thorough nursing assessments and create individualized care plans.
• Coordinate schedule with Hospice Director of Nursing and participate in on-call rotation.
• Ensure care aligns with Castle Hospice policies and procedures.
