Job summary

Allegiant Hospice LLC is seeking a compassionate Registered Nurse to join our hospice team as an RN Case Manager. This full-time, community-based role serves patients in private homes and care communities throughout Southeast Wisconsin, including Milwaukee.

You will perform case management visits, assess patients, coordinate care, and educate families. Hospice experience is welcome but not required, with training provided.
