Castle Senior Living, Inc. Hospice is now seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) for our new Hospice program.

Formación

• Current certification as a Registered Nurse with the state of Wisconsin.
• Exceptional communication, problem solving skills, and a compassionate bedside manner.
• Valid driver’s license and automobile insurance.

Responsabilidades

• Provide skilled nursing care and support to patients and families where they call home.
• Perform thorough nursing assessments and create individualized plans of care.
• Coordinate schedule with Hospice Director and participate in on-call rotation.
