Job summary

UnitedHealth Group seeks an Afterhours Registered Nurse for Northern Arizona Hospice, part of LHC Group, to deliver comprehensive hospice care after regular hours. The role includes triage by phone, afterhours visits, and evaluating eligibility within 48 hours.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current CPR Certification.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to dependable transportation.

Responsibilities

• Triage patient needs over the phone until an in-person visit by a nurse.
• Respond promptly to afterhours crisis calls with urgency.
• Make initial nursing evaluation to determine hospice eligibility within 48 hours.
• Perform afterhours visits including admissions, deaths, recertifications, and routine visits.
• Assess patient/family needs and reassess at least every 15 days.
• Document problems, goals, interventions and responses to care.
