Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a compassionate Hospice RN Case Manager to join our Pittsburgh team. You will provide in-home, patient-centered nursing care, develop individualized hospice plans, and collaborate with an interdisciplinary team to honor patient goals.

Schedule is Monday–Friday, 8:00 am–5:00 pm with opportunities for on-call rotation. Benefits include health insurance, 401(k), PTO, mileage reimbursement, and tuition discounts, among others.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Must be a licensed driver with an automobile that is insured and in good working order
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patients wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
