Job summary

Adoration Health in Chattanooga, TN seeks a compassionate RN Case Manager - Hospice to deliver patient-centered care in home settings. You will assess physical, emotional, and spiritual needs, create individualized hospice care plans, administer medications and therapies, monitor progress, and educate families to support comfort and dignity at end of life.

This full-time position offers a predictable schedule (Monday–Friday, 8am–5pm) with on-call rotation as required, a comprehensive benefits

Qualifications

• Graduate of an accredited nursing program with current RN licensure in state of operation.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured vehicle.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
