Job summary

Adoration Health is seeking a dedicated Registered Nurse Case Manager for hospice services in Prattville, AL. The role requires providing comprehensive patient-centered care, coordinating treatments, and supporting families in a compassionate home-based setting.

Responsibilities include developing care plans, administering medications, managing symptoms, and collaborating with a multidisciplinary team. Strong clinical/d documentation skills and valid state licensure are essential.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient's family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
