Job summary

Adoration Health in Manchester, TN, is seeking a Registered Nurse Case Manager for Hospice. This role provides expert, patient-centered hospice care in a team-based environment, with a schedule of Monday through Friday, 8am-5pm.

You will assess and monitor patients, create care plans, administer treatments, educate families, collaborate with interdisciplinary staff, and maintain timely documentation. Eligible candidates have RN licensure and hospice experience; driving with a valid license is

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
