Job summary

St. Mary’s Home Care and Hospice in Bucks County, PA, seeks a dedicated Hospice RN Case Manager to provide professional nursing care to hospice clients in their homes, in line with physician orders and licensure.

You will assess, plan, implement and supervise care, collaborate with the interdisciplinary team, attend weekly meetings, and coordinate with the patient’s physician to manage symptoms. We offer generous PTO, growth opportunities, day-one benefits, mileage reimbursement, tuition

Qualifications

• Graduate of an approved nursing education program.
• PA RN license.
• Minimum 2 years as professional nurse in acute care, home care or ER.
• Home care experience preferred.
• EPIC experience preferred.

Responsibilities

• Provide professional nursing care to hospice clients as prescribed by physicians.
• Assess, plan, implement and supervise hospice patient care and outcomes.
• Serve as case manager, collaborating with the interdisciplinary team.
• Coordinate with the primary care physician and hospice medical director as needed.
