Job summary

The Care Team Home Health & Hospice in Midland, MI is seeking an experienced Registered Nurse Case Manager (RNCM) to deliver high-quality hospice care. You will conduct comprehensive patient assessments, develop individualized plans of care, and coordinate with physicians, caregivers, and the interdisciplinary team.

Standard hours are Mon–Fri, 8:00 AM–4:30 PM with potential on-call rotation and regional travel within a 1-hour radius around Midland.

Qualifications

• Active RN license
• Hospice or home health experience preferred
• Strong clinical assessment and care coordination skills
• Ability to work independently in the field
• Strong communication and documentation skills
• Experience using HCHB (Homecare Homebase) preferred

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care
• Provide skilled nursing care in accordance with hospice standards and physician orders
• Coordinate care with physicians, caregivers, and interdisciplinary team members
• Educate patients and families on symptom management, disease progression, and end-of-life care
• Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB (Homecare Homebase)
• Participate in interdisciplinary team meetings and care planning
• Support after-hours patient needs as part of an on-call rotation when applicable
