Job summary

The Care Team Home Health & Hospice in Midland, MI is seeking an experienced Registered Nurse Case Manager (RNCM) to join our hospice team. You will conduct comprehensive patient assessments, develop individualized plans of care, and coordinate with physicians, caregivers and the interdisciplinary team to ensure high-quality patient care.

This role offers a Monday–Friday schedule with on-call rotation in some branches, mileage reimbursement, and a collaborative environment focused on quality,

Qualifications

• Active RN license.
• Hospice or home health experience preferred.
• Strong clinical assessment and care coordination skills.
• Ability to work independently in the field.
• Strong communication and documentation skills.
• Experience using HCHB preferred.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care
• Provide skilled nursing care in accordance with hospice standards and physician orders
• Coordinate care with physicians, caregivers, and interdisciplinary team members
• Educate patients and families on symptom management, disease progression, and end-of-life care
• Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB (Homecare Homebase)
• Participate in interdisciplinary team meetings and care planning
• Support after-hours patient needs as part of an on-call rotation when applicable
