Job summary

The Care Team Hospice is seeking a Registered Nurse Case Manager to provide skilled nursing care and coordinated end-of-life support to patients in their homes and facility settings while collaborating closely with the interdisciplinary team. This role covers the Midland, MI area within a one-hour radius and emphasizes compassionate, patient-centered hospice care.

Standard hours are Monday through Friday, 8:00 AM to 4:30 PM, with opportunities to participate in an on-call rotation for additional

Qualifications

• Licensed Registered Nurse with hospice experience
• Excellent interpersonal and communication skills
• Ability to work in a team-based interdisciplinary environment

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care
• Provide skilled nursing care in accordance with hospice standards and physician orders
• Coordinate care with physicians, caregivers, and interdisciplinary team members
• Educate patients and families on symptom management, disease progression, and end-of-life care
• Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB
• Participate in interdisciplinary team meetings and care planning
• Support after-hours patient needs as part of an on-call rotation when applicable

Schedule, Hours & Territory

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Up to a 1 hour radius around Midland, MI
• RN Case Managers may participate in an on-call rotation with additional pay
• Some branches offer dedicated on-call staff, resulting in backup on-call coverage only
