Job summary

Adoration Health is seeking a Registered Nurse Case Manager to join our Hospice team in Cleveland, TN. You will assess/monitor patients’ physical, emotional, and psychological needs, create hospice plans, administer medications, and educate families while collaborating with an interdisciplinary team.

This full-time role offers a Monday–Friday schedule, 8am–5pm, with coverage area Bradley and Polk Counties, and a supportive environment focused on clinical excellence and compassionate,

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient's family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
