Job summary

Adoration Health is seeking a Hospice Registered Nurse to join our team in the Indianapolis area. This PRN role involves providing 1:1 hospice care at patients’ homes, creating care plans, administering treatments, and supporting families.

Travel across the territory is required, with clinical autonomy and team collaboration. The ideal candidate will have RN licensure, at least 1 year of nursing experience (3+ years preferred), and a passion for end-of-life care within a compassionate,

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile insured per state/organization requirements

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with patients' wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
