Job summary

Ennoble Care is hiring a full-time Hospice RN Case Manager who resides within or near Montgomery County, PA. You will work with the interdisciplinary team to provide skilled nursing assessments, planning, and care to maximize patient comfort and support families.

The role requires a PA RN license, 1–2+ years of hospice experience, and the ability to coordinate care across settings. A valid driver's license and reliable vehicle are expected for home visits and travel within the territory.

Qualifications

• Valid PA RN license.
• 1–2+ years of professional nursing experience, preferably in hospice.
• Ability to assess needs of patients and caregivers across settings.
• Skilled nursing practice and coping with emotional stress.
• Ability to write comprehensive, accurate reports.
• Team player in an interdisciplinary care model.
• Valid driver's license and reliable vehicle.
• Ability to work independently within a multidisciplinary team.

Responsibilities

• Assess, plan, implement, and evaluate patient care across all nursing phases.
• Document notes in the EMR accurately and timely.
• Coordinate care with hospice staff, physicians, and other agencies.
• Educate families on levels of care and symptom management.
• Participate in patient care conferences and staff discussions.
• Oversee NAC documentation biweekly and during off-hours as needed.
