Ennoble Care is hiring a full-time Hospice Registered Nurse Case Manager who resides in or near Somerset County, NJ. The RN Case Manager conducts skilled nursing assessments, plans care, and coordinates with physicians and the interdisciplinary team to maximize patient comfort and support families.

Responsibilities include managing patient caseloads, communicating with attending physicians, documenting in EMR, participating in group meetings, and providing on-call coverage when needed.

Formación

• Must hold a current NJ RN license.
• At least 2 years of professional nursing experience with hospice preferred.
• Ability to assess patient and family needs in varied settings.
• Excellent communication and documentation skills in EMR.

Responsabilidades

• Manage an assigned patient caseload including assessment, planning, implementation, and evaluation.
• Coordinate care with attending physicians, Hospice staff, and other agencies.
• Maintain regular communication with physicians and family caregivers.
• Provide timely documentation in electronic medical records.
• Participate in Interdisciplinary Group meetings.
• On-call coverage as requested.
