Job summary

Moments Hospice is seeking an RN Case Manager to provide high-quality end-of-life care on a mobile, on-road schedule in Minnesota. You will assess patients, plan and deliver nursing care, and coordinate with families and the health team to ensure comfort and dignity for patients.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with EMR software.
• Excellent planning and communication skills.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
