Job summary

Moments Hospice in Saint Cloud, MN is seeking an experienced RN Case Manager to coordinate compassionate patient care across the final days of life. In this field role you will assess needs, develop care plans, educate families, and work closely with our clinical team to ensure comfort and relief.

Qualifications

• Licensed as a Registered Nurse / RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
