Qualifications

• Active RN license in good standing.

Responsibilities

• Manage plan of care from referral through discharge for hospice patients.
• Perform initial nursing assessment to determine hospice needs.
• Provide complete physical assessment and medical history.
• Collaborate with patient/family to implement care plans with goals.
• Coordinate medications, equipment, and services with cost management.
• Administer medications and document responses; report adverse reactions.
