Ascend Hospice & Palliative Care MA is hiring a Hospice Registered Nurse Case Manager to support on-call coverage and patient visits, with assessments and care planning in collaboration with the healthcare team.

The role focuses on delivering compassionate end-of-life care, ensuring a continuous, well-coordinated patient experience. Requires MA RN license, travel willingness, and strong communication skills.

Qualifications

• Associate's Degree in Nursing required.
• Must have a State Certified RN license.
• 2 years' experience as a professional nurse, hospice experience preferred.
• Must have a valid Driver's license and reliable transportation.
• Must be willing to travel.
• Strong verbal and written communication skills.

Responsibilities

• Complete an initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluate the patient’s nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient and family to direct care and ensure continuous, well-coordinated services.
• Counsel patient and family in end-of-life needs.
• Initiate appropriate preventative and palliative nursing procedures.
• Communicate with the physician regarding patient needs and changes in condition.
• Supervise the Hospice Aides.
