Job summary

Integritus Healthcare, Inc. in Pittsfield, MA seeks a compassionate Hospice Aide to provide direct patient care and support to hospice patients in-home and in the field. The role requires current MA CNA or HHA certification and a minimum of one year CNA experience.

Qualifications

• One year of CNA experience is required.
• Current MA CNA or HHA certification is required.
• Ability to provide personal care per plan of care.

Responsibilities

• Provide direct patient care within the CNA scope.
• Assist with personal care per individualized plan of care.
• Maintain a clean and safe home environment.
• Support nutritional needs and feeding per plan of care.
• Notify RN/case manager of changes in patient condition.
